Employment Opportunity Posting: Early Intervention Part-time Physical Therapist, La Plata, San Juan, Montezuma/Dolores and/or Archuleta counties.
Community Connections, Inc. is seeking a part-time licensed Physical Therapist to provide PT services for children birth - 3 in Early Intervention. Flexible schedule and hours may range from 5-15 hours per week. Services are provided in homes and community settings, both in-person and via Zoom/telehealth. We promote a coaching model (empowering parents and teachers to continue implementing strategies between visits). Qualified applicants must delight in working with young children and enjoy being part of a team supporting families. Ability to provide services in Spanish is ideal, but not required. Intimate knowledge of Indigenous (primarily Dine and/or Ute) or Hispanic cultures is also ideal but not required. $43.25/hr (plus mileage) includes direct service as well as prep and travel time. The Organization
The Case Management Agency is comprised of four main components: Administration, Case Management, Early Intervention Services, and Family Support Services. The Board of Directors establishes financial, program, and personnel policies that in turn are administered by the President/CEO. The Board may have up to eleven members representing businesses, parents of individuals with disabilities, people with disabilities, etc. All board members are volunteers from communities within our service delivery area which includes Archuleta, Dolores, LaPlata, Montezuma, and San Juan counties. A continuous attempt is made to have representation from all ethnic and cultural communities served.
