Early Intervention Speech Language Pathologist, La Plata, San Juan,  and/or Archuleta counties

Community Connections, Inc. is seeking a part-time licensed Speech Language Pathologist to provide SLP services for children birth – 3 in Early Intervention. Flexible schedule and hours may range from 2-16 hours per week. Services are provided in homes and community settings, both in-person and via Zoom/telehealth. We promote a coaching model (empowering parents and teachers to continue implementing strategies between visits). Qualified applicants must delight in working with young children and enjoy being part of a team supporting families. Ability to provide services in Spanish is ideal, but not required. Intimate knowledge of Indigenous (primarily Diné and/or Ute) or Hispanic cultures is also ideal but not required. $35-40/hr DOE (plus mileage)  includes direct service as well as prep and travel time. Title: Early Intervention Therapist
 
Reports To: Director of Children and Family Services
 
FLSA:  Part-time/Hourly/Non-exempt
 
Job Description: Early Intervention Therapists serve Community Connections as part-time employees working with children and families through the Part C/Early Intervention Program.  Schedules are flexible, and are generally scheduled with families and coordinated with the Child Find team as needed.  Participation in needed Part C trainings is required.  Must provide own transportation and be willing to travel around local communities to families’ homes, childcare settings and other community locations where children and their families are. 
 
Responsibilities:
  • Participate in public and private insurance billing as requested and as able.
  • Become a Medicaid provider.
  • Assist in developing Individual Family Support Plans (IFSP).
  • Implement assigned strategies and action plans from the IFSP.
  • Work with the Service Coordinator for each family to identify family priorities and gather resources for the family.
  • Participate in trans-disciplinary teams as needed.
  • Attend IFSP meetings and reviews.
  • Provide ongoing assessment, at least annually, with a qualified tool and family input. 
  • Maintain current and accurate documentation, including certifications, licensing, endorsements and registrations, and shall register and update his or her information at least annually in the provider database maintained by the division for developmental disabilities.
  • Participate in the reporting of Child outcomes, meeting the minimum requirements. 
  • Submit paperwork for billing and payroll in a timely and accurate manner.
  • Stay current in trends in Early Intervention, Best Practices and Evidence-Based Practices for applicable field.
  • Respond to families’ needs in professional and courteous manner, building relationships with families and children that will become the basis for all interventions.
  • Communicate problems or concerns to the Service Coordinator or Early Intervention Program Manager immediately.
  • Fulfill other duties requested by the Early Intervention Services Director to assure community compliance with the State and Part C Federal Assurances.

Qualifications: Must hold a B.A or Master’s in the appropriate professional field (Occupational Therapy, Physical Therapy, Speech and Language Pathology, Early Childhood Special Education, Social Work, etc.) and corresponding licensure for the State of Colorado (SLP: Colorado state certification or graduate student in speech-language pathology completing their clinical fellowship.)  Experience working with children birth through three strongly preferred.

While performing the duties of this job, the employee is regularly required to be stationary, move indoors and within the community, position self, use objects, communicate verbally, in writing, and via in-person, computer and phone, and observe. The employee will be required to attend meetings, training, and other community activities within the region of Southwest Colorado, with the possibility of occasional travel within Colorado or out of state to training or conferences.

Compensation: $36.05 - $42.40 per hour
